Application Process

All of the following are required to be complete for your application to a PGY-1 or PGY-2 pharmacy residency at Shands Jacksonville:

• Residency application
• Letter of intent (address to program director, typically about one page in length)
• Curriculum vitae
• Three (3) letters of reference from professional sources (using the attached form, often accompanied by a letter also)
• Official college transcripts (for pharmacy coursework)

Application materials for all programs except Emergency Medicine should be submitted through PhORCAS. For the Emergency Medicine PGY-2 program, please visit that program's page.
